This comprehensive book sheds light on the complex world of medical records. Although the book originally was intended to focus just on medical records, each author took advantage of the opportunity to provide a wider glimpse into the subject matter. Thus, the reader will come away from this book gaining broader knowledge about healthcare delivery, which will assist in analyzing medical records and details. The text is intended to benefit attorneys, paralegals, legal nurse consultants, risk managers, and anyone else with a keen appreciation of the nuances of medical records.

The text is divided into four sections. Part One provides an overview of documentation practices and legal issues associated with generating, preserving, and obtaining medical records. Part Two and Part Three bring the reader into the specialty areas of inpatient and outpatient medical records. Part Four gathers together chapters on forensic issues of medical records. the final section deals with the impact of records on litigating cases.
